Decide what you want the check-up to do
Apparently health has become a trend here in Bangkok—a good “healthy” trend of course. And with that comes the proliferation of wellness products and health-checkup packages, which begs a certain recurring question: which is the best?
For me personally, an impossible question to answer (like how do you actually know?). So I do what any rational person would: answer a question with another question. What do you want out of it?
Different answers buy different things. Screening means finding something in time to know about it. Optimization would require a much more aggressive (or maybe like proactive) approach.
Choose what you want before comparing the price
Use this question list to compare providers as an example.
| Choices | What to compare | The question to settle first |
|---|---|---|
| Hospital package | Physical examination, laboratory tests, imaging, and other charges outside the package | If a result needs another specialist or procedure, what will that cost? |
| Clinic or laboratory package | The exact tests and whether a clinician consultation is explicitly included | Who interprets the results, and is that consultation part of this quote? |
| At-home testing | The draw, any visit fee, clinical review, physical assessment and arrangements for tests done elsewhere | Which parts happen at home, and which require a separate appointment? |
Preventive check-ups differ from appointments for sickness or injury.[1] If you have new or persistent symptoms, arrange a clinical assessment rather than relying on a routine screening bundle to investigate them.
You can bring either answer, screening or optimizing, to any of the three choices above.
Who the package was built for
The core packages have been the same for quite some time. They run from a small check-up to an absolutely gigantic list of things to test for.
The minimum inside each package is usually designed around a population target: what public health research concluded everyone in that population should at least test for, meant to catch the early progress of active disease in time.
So if your answer was screening, the smallest packages are probably enough.
General health checks have limitations.[6] In a 2019 Cochrane review, general multi-disease health checks in unselected adults had little or no effect on death from any cause.[6] That finding concerns broad checks in the studied populations; it is not a verdict that all preventive care or symptom-driven testing is useless. What we do with the results after matters.

Four prices for four population targets. If you came for screening, read the contents column first. Does the shortest list here already carry what a screening check is meant to carry? If it does, the question is why the rest? What diseases are they screening for? How accurate are they? What happens next if any of the tests comes back abnormal?
Ask what each test would change
For example, three of the four packages above print an electrocardiogram (ECG).
For one concrete example, the US Preventive Services Task Force recommends against screening with resting or exercise ECG to prevent cardiovascular events in adults without symptoms or known cardiovascular disease who are at low cardiovascular risk.[9] It finds insufficient evidence for routine screening at intermediate or high risk; this is different from using an ECG to investigate symptoms.[9]
So ask your clinician: “If this result is abnormal, what would we do next?”

What if it misses something
The pushback to the minimum package is this: what if it misses something?
A 2021 review found associations with more chronic-disease recognition and greater use of preventive services, alongside generally no reduction in mortality or cardiovascular events.[7] A 2026 systematic review reported a similar distinction: randomized trials generally did not show statistically significant mortality or cardiovascular-event reductions, while some observational studies suggested protective associations.[8] Those designs should not be treated as equally strong evidence that a package causes a benefit.
A checkup is just one point in time, a cross-sectional snapshot of the biology you have at that moment, of your body in that moment alone. What medicine does is draw from its statistical correlation results from existing studies to try to make a correlation with what that means to you, or rely on the knowledge that some biomarkers may persist in that stage longer than others. For example, HbA1c reflects about 3 months of your blood sugar status, so we at least could take comfort in knowing that.
This is one of the homework assignments that anyone needs to know before doing a blood test. We need to know how long this snapshot would be relevant for each biomarker because every substance is unique and has a different half-life or different clearance from the body.
The minimum packages usually cover the major biomarkers that would signify early onset of a major disease.
Like, the anxiety or the FOMO is there. But it is not backed by the actual decision or awareness of what these tests could lead to if something comes back out of range. This is not an accusation. It's just testing for more without knowing the context or the why behind it, which would only add to the decision burden. This can lead to even more anxiety, more worries, or more of that.
An abnormal screening ECG can lead to further testing with its own potential harms.[9]
It would be best if you make an informed decision right from the start. If you ask yourself whether you have the time to figure out the why behind each test right here and now, then yeah, go for it. Make an informed decision and pick the biomarker that relates to the condition that you are looking out for. If not, the minimum packages would be best.
If you came to optimize, the answer gets more nuanced. The deficiency varies from person to person. You have to learn what each biomarker means and which disease it is trying to tell you about.
The decision
Screening choices depend on age, sex, overall health, family history and risk.[1] Pick the minimum that fits yours.
If you are optimizing, no package fits. For anything you don't know, you can always educate yourself more about what should go into a health checkup for you and why.
And you need to calibrate your expectation. Medical doctors in hospitals, their job is basically to catch disease, not to make an analysis or research specifically just for you. Their work is to catch and/or manage disease that is found on that check-up, according to guidelines written for the entire population, not just for you.
You have to be aware of a potential mismatch: when you pay a premium price maybe you feel like there is going to be a whole research team behind you and everyone is going to work their hardest to find out what the best way forward for you is. More often than not that is not the truth.
A checklist to send with your quote request
Copy this into your message to the provider:
- [ ] Which tests does the clinician consider appropriate for my history and risks?
- [ ] What is the exact package name, and am I eligible for this price?
For the practical home-draw process, read our at-home blood-testing guide for Bangkok.
"Which is the best?" What you want out of it, and you have to calibrate and match what you want out of it. Someone has to know what to do when these biomarkers come back out of range. More often than not that person needs to be you.
Considering at-home care with Orba?
Optimization done properly is a full-time job. That is why Orba exists.

Compare named packages rather than the phrase “full body.” Our checked examples include a ฿2,900 male laboratory bundle and a ฿16,300 male hospital Executive package with physician review and imaging.[5][3] They cover different services. These examples do not establish a citywide price range; eligibility, chosen tests and additional care affect your quote.
